Inder Paul Singh, MD, a glaucoma surgeon and the president of The Eye Centers of Racine and Kenosha in Wisconsin, was eager to try Alcon’s new Voyager Direct Selective Laser Trabeculoplasty (DSLT) system at his practice, because data indicate that SLT is a better first-line treatment for most glaucoma patients than topical drops. He maintains that DSLT is a win-win for patients and physicians. “SLT can’t be used on some patients who have angles that are difficult to view,” he says.

In ophthalmology, the administration of topical medications is a cornerstone for glaucoma management. However, commercially available drop sizes (30+ µL) exceed the eye’s capacity to retain the medication (7-10 µL), leading to excessive dosing, overflow, periorbital irritation, and drainage by the nasolacrimal ducts causing systemic absorption. The increased risk of systemic adverse effects (AEs), particularly with drug classes like beta-blockers, has additional implications for patient safety.

How does one treat glaucoma? Of course, the only proven therapy at this point in time is lowering intraocular pressure (IOP). My favorite answer to this question has always been that the goal of glaucoma treatment is to maintain the patient’s vision-related quality of life without decreasing the patient’s quality of life. With this goal in mind, the practical answer is to lower IOP in a way that maximizes the patient’s overall quality of life.
