In May 2023, I visited South Central Virginia as part of a conference series. When our group toured a community hospital, I learned that two of the three hospitals along the nearby Virginia-North Carolina border no longer provided perinatal care. 1, 2 The two labor and delivery (L&D) units closed within 18 months of one another, leaving three counties, 144,000 people, and more than 2,100 square miles with only one delivery location.

To the Editor: Swartz and his colleagues bring concerns regarding methods of our analysis and methodologies.1 In our systematic review of the literature pertaining to the harms of screening colonoscopy, we judged that a meta-analysis was not statistically sound due to the very high heterogeneity of study results (I2 of 97%).

ConclusionUnderstanding and reducing harm is a common goal of clinicians and health systems.19 We conducted a rigorous analysis of harms that occurred in the course of screening colonoscopy, which is the third most common cancer screening procedure in the US, exceeded only by Papanicolaou tests and mammography.20 Recently, a publication in the New England Journal by Bretthauer et al. suggested that the mortality benefits of screening colonoscopy have been overestimated in cohort studies.
