Q: The facility I work in has many new team members. How can the director of nursing services best prepare our staff for survey? A: Staff who understand what to expect during surveys can avoid unwanted comments or actions. Often, the facility’s Statement of Deficiencies, where surveyors list findings, includes a surprise related to something staff said or did. To avoid unwanted surprises, devise a plan to educate staff on the survey process, beginning upon hire and continuing monthly.

Q: When should the facility conduct a root cause analysis, and when should we conduct a performance improvement plan (PIP)? A: Issues that have the potential to cause harm may prompt a root cause analysis (RCA). Examples include medication errors, falls, bruises, fractures, staff turnover and elopements. While an RCA is often the first step of a PIP, the two are different processes. RCA is a systematic analysis that helps uncover vulnerabilities by identifying the underlying causes of a problem.
