In Reply We thank Adler et al for their insightful comments regarding our article1 and the treatment of lepromatous leprosy. As highlighted by these authors, the US National Hansen’s Disease Program (NHDP) has traditionally recommended treating multibacillary leprosy with daily rifampin, dapsone, and clofazimine for 24 months. However, as the authors mention, there have been recent new recommendations for monthly, rather than daily, multidrug leprosy therapy.

A man in his 30s with no relevant medical history presented to the dermatology clinic with a 3-year history of progressive nodules affecting his face and forearms. He also noted a 1-month history of blurry vision in his left eye. He was born in Micronesia and moved to the US in 2018. He had no close contacts with leprosy or individuals with skin conditions, and no other household members were affected. He did not have any notable animal exposures.

The Diagnosis: Majocchi GranulomaHistopathology showed rare follicular-based organisms highlighted by periodic acid–Schiff staining. This finding along with her use of clobetasol ointment on the hands led to a diagnosis of Majocchi granuloma in our patient. Clobetasol and crisaborole ointments were discontinued, and she was started on oral terbinafine 250 mg daily for 4 weeks, which resulted in resolution of the rash.
