Recently, the international company Hologic has withdrawn fetal fibronectin (fFN), a commonly used biomarker for preterm birth prediction. This is related to quality controls and sourcing consumables. However, fFN is currently recommended by the National Institute for Health and Care Excellence (NICE) and the National Health Service in England and had been implemented widely across Europe and the United Kingdom.

The rate of cesarean section has been increasing globally over several decades. Between 1990 and 2014, there was an absolute increase in cesarean section rate of 12.4%, translating to 1 in 5 women delivering by cesarean section worldwide [1]. In England, over 1 in 3 women deliver by cesarean section, and 24% of all deliveries are by emergency cesarean section, of which 5% are performed on women at full cervical dilatation [2,3].

Worldwide in 2020 a baby died every 40 seconds because of complications of prematurity. Preterm birth is the leading cause of death among children under 5 years old. The burden of preterm birth is heavier in in low- and middle-income countries, where around 80% occur. The countries with the highest recorded preterm rates in Africa are Malawi, South Africa, Ethiopia, the Democratic Republic of Congo and Botswana. In Ethiopia 12.9% of babies were born preterm in 2020. In Nigeria the figure was 9.9%.
