Recent advancements in targeted therapies, including selective inhibitors of key inflammatory pathways, offer promising outcomes for patients with active psoriatic arthritis (PsA). At ACR 2024, there have been further developments in novel therapies in PsA. Three innovative treatments: Zasocitinib, Vunakizumab, and Sonelokimab, as detailed in recent phase 2 trials, were presented at ACR 2024. Zasocitinib (TAK-279) is a selective oral TYK2 inhibitor and has shown robust efficacy in phase 2b trials.

At last year’s ACR, I shared about the early clinical trial data on bimekizumab in psoriatic arthritis. Bimekizumab (BKZ), a monoclonal IgG1 antibody that selectively inhibits interleukin (IL)‑17A and F. There have been more updates on the 2 year data on BKZ at #ACR24, and this is a summary of the studies. In Abstract 0602, BKZ demonstrated sustained improvement across GRAPPA domains, including peripheral arthritis, axial disease, enthesitis, and dactylitis, up to two years.

I am a big fan of animals and wildlife. Whenever I can, I try to wander outdoors to look for our friends big and small in the animal kingdom. If it rains, as often it does in the UK, the latest edition of Planet Earth will do. So, I was naturally attracted to the presentation by Professor Iain McInnes from the University of Glasgow, oral presentation OP0195 at #EULAR2024 where the link between camels and psoriatic arthritis was made.

Extra-musculoskeletal manifestations (EMMs) occur in patients with spondyloarthritis. The prevalence of EMMs such as uveitis, psoriasis and inflammatory bowel disease are 26%, 9% and 7% respectively in axial spondyloarthritis (axSpA). The prevalence of subclinical inflammatory bowel disease (IBD) is higher at 50% when screening was done by ileocolonoscopy and biopsy or endoscopic capsule, showing a 10 fold higher co-existence between IBD and axSpA.

Psoriatic arthritis (PsA) can affect up to 30% of patients with psoriasis (PsO). Understanding the factors that predispose patients with PsO who progress to PsA is the first step in understanding how early interventions in PsO may augment the progression to PsA. The majority of patients with PsA present with preceding PsO. At #EULAR2024, there are a few presentations on this topic that will help us understand this progression from PsO to PsA better.

Guidelines (ACR/EULAR/GRAPPA) for the management of psoriatic arthritis (PsA) recommend the early referral of patients with the suspected condition for early assessment and treatment. There remains a diagnostic delay of 1-2 years in PsA and ideally treatment should be commenced within 1 year of symptom onset. How early is early in PsA and is there a window of opportunity for treatment in PsA to ensure optimal outcomes?
