We read with interest the article “Myopic Traction Maculopathy in Fovea-Involved Myopic Chorioretinal Atrophy” by Lee et al. [1]. The authors present important findings suggesting that MTM may exacerbate atrophy progression and that early surgical intervention could help mitigate this. These conclusions need careful consideration in light of the significant variability in anatomy and pathology seen in these patients.

1. IntroductionEctasia after penetrating keratoplasty (PK) is a late complication that can occur after the procedure, characterized by the thinning and steepening of the cornea at the donor–recipient junction [1] (Figure 1), potentially leading to high irregular astigmatism and significant visual impairment. It can occur years or decades after PK is performed for any indication, but it is relatively rare in non-keratoconus eyes [2,3,4,5].
