Toxic/metabolic encephalopathies can sometimes be mistaken for NCSE. Generalized periodic discharges (≤2.5 Hz) represent a challenging diagnosis, and electroencephalographers must rule in favor of either acute encephalopathy or NCSE. Due to self-perpetuating processes and the failure of self-terminating mechanisms, NCSE becomes unlikely to cease transitorily after external stimulation.
