To address significant gaps in testing for hepatitis C virus (HCV) we need a better understanding of who and what is missing from elimination efforts. Previously relied upon descriptions of ‘who’ was missing are insufficient. We also need to understand the ‘where’ and ‘what’ of service design and delivery. Eliminating hepatitis C is a challenge of social and structural change to ensure that services and systems do not miss the needs of the people they seek to serve.

When Geoff* left prison after his sentence ended, he was told he would be provided with help to return to the community and get on with this new chapter in life. The result was sadly predictable. Geoff was unable to access public housing due to a lengthy wait list and he soon found himself rotating between staying with friends or at hostels and living on the street.

This analysis explores variation in how OAT providers relate their work in deimplementation to concepts of social inequity in health during the COVID-19 pandemic. Interviews were conducted with 29 OAT providers in New South Wales, Victoria, Queensland, Western Australia and Australian Capital Territory (59% doctors, 31% nurses, 10% service managers; average number of years working in OAT was 11).
