Over my 21 years of using opioids I’ve often joked to myself that I don’t have a drug problem, I have a running-out-of-money problem. In the mid-aughts when I was buying Opana (oxymorphone) and Dilaudid (hydromorphone) off the street, my tolerance was under control in the sense that the pills were working fine for me. Because they’re a sure thing. As long as you’re dealing with legitimate pharmaceuticals, you know what you’re getting every time.

Since early 2023, I’ve been surfing my way across what feels like every New Jersey substance use treatment facility that takes Medicaid. In that time I’ve worked my way down from 140 mg methadone per day to 60 mg. I’m about to check in to lucky number 13, which will hopefully be the last one before I get down to 30 mg and make the jump to buprenorphine.

Why do people drink their own piss? I’ve often wondered if childhood fears reveal something deep about our psyche; whether our adult choices are somehow wrapped up in them. When I was a child, I was fascinated and horrified by stories of people stranded in deserts. Someone would be driving along a long road and their engine would fail. They would be stuck, torn between leaving the vehicle to find help and following the advice to stay with it. My primary fear was the unquenchable thirst.
