Robust evidence has shown that crystalloid fluid expansion is beneficial to patients in circulatory shock in the first 6 h of their trauma. However, central clinical questions yet remain to be answered: Which clinical and/or paraclinical parameters should guide fluid initiation? In which context should crystalloid fluids be administered? In the prehospital setting? Which crystalloids should be used? Should they be continued upon hospital admission? How much fluids should be administered?

We thank Dr. Piazzola and Prof. Castinetti for their insightful comments (1) on our study on long-term treatment of 36 patients with Cushing disease (CD) not cured by transsphenoidal surgery (TSS) (2). Drug treatment (DT) was initiated in 29/36 patients but it proved a long-term efficient treatment in only 38% (11/29). We agree that these results are provocative: How can we explain that DT was discontinued so frequently?
