    1 INTRODUCTION Clinicians typically rely on intermittent sphygmomanometer readings to identify dysregulated postural blood pressure, observing sporadic changes when an individual moves from a seated or supine position to standing. However, the current consensus for diagnosing initial orthostatic hypotension necessitates the use of continuous measurement because it requires blood pressure to be captured within 15 s of standing upright.

    Polymyalgia rheumatica (PMR) is typified by pain and stiffness of the shoulder and hip girdles. Previous qualitative studies have highlighted the importance to patients of additional symptoms, including fatigue and sleep disturbance. These features of PMR are less recognized by clinicians and have not yet been quantified in the literature. Further, the adequacy of the current treatment paradigm in addressing these symptoms is unknown.

    Last week, the FDA approved sarilumab, a fully human monoclonal antibody that inhibits binding of IL-6 to its α receptor, for the treatment of glucocorticoid-resistant polymyalgia rheumatica (PMR). For patients living with this chronic rheumatic disease and ardent PMR researchers like me, the magnitude of this development cannot be understated.  Among inflammatory autoimmune rheumatic diseases, PMR has an incidence second only to rheumatoid arthritis (RA) in terms of its lifetime risk.
