History of PresentationA 43-year-old man presented to the emergency department with shortness of breath of 6 months’ duration. He previously played ice hockey regularly, but now experienced NYHA functional class III dyspnea. He had no associated chest pain, cough, orthopnea, or paroxysmal nocturnal dyspnea. His blood pressure was 114/82 mm Hg, heart rate was 105 beats/min, and oxygen saturation was 94% on room air.
