DiscussionOur study analysed health service data to examine the Community Care programme’s enrolment factors and patients’ characteristics. Over a 12-month period between 2016 and 2017, the Community Care Programme more frequently served an older, high healthcare needs population, with chronic conditions like chronic obstructive pulmonary disease, obesity and diabetes. However, individuals in residential aged care homes are under-represented, suggesting scope for improved outreach.

This design is a systematic method of coding text in order to categorise similar words and sentences [10].
