Imagine a medical syndrome so common that as many as one in five patients who nearly die in a medical setting experience it. Also, imagine that this syndrome has a profound, lifelong effect on both the patient and their family. Finally, imagine that if this syndrome is mishandled by the patient’s physician, it may have major adverse consequences for the patient and their loved ones. Sounds like something all physicians and medical personnel should be able to recognize and treat, doesn’t it?

Privacy protection does not cease to exist at death, as privacy laws protect information until 50 years post-mortem. The hospital is like a river. Patients float downstream into our care, stay in sight for a moment, and leave after we have done everything we can to help them. Regardless of why they arrive or how long they stay, every person in the hospital will always leave, whether for recovery, rehabilitation, transfer, or death. Meanwhile, the hospital staff guard the river banks, keeping watch over the steady flow of patients that happen to travel down their portion of the river. It is Tuesday morning.

It has been only two weeks since I realized the job I had was gone. As physicians, I think we have this idea of what our work will look like into the future. It is enshrined in pomp and circumstance from the start as we take an oath to “do no harm,” it’s encrusted with an unspoken culture of medicine that often asks us to give more than we thought possible. We build systems, evaluate quality metrics, and find ways to get through global pandemics all while trying to embrace change.
