Just 40 years ago, the prevalence rate of obesity in most high-income countries was <5% to 10%, whereas the prevalence rates of smoking, high blood pressure and high cholesterol levels were much higher. Consequently, rates of myocardial infarction and stroke were substantially higher, and most patients diagnosed with heart failure (HF) had a short life expectancy, many succumbing to disease-related cachexia (Figure).
