Helping Dual-Eligible Individuals Navigate Medicare and Medicaid Nearly 13 million US residents are dually eligible for and enrolled in both Medicare and Medicaid. They are among the nation’s most medically, socially, and financially at-risk individuals, comprising adults with low income who are living with disabilities, dementia, severe mental illness, multimorbidity, and functional limitations.

In the past decade, Veterans Health Administration (VA) spending on community care more than doubled from approximately $8 billion in 2014 to over $18 billion in 2021. A recent study that I coauthored with colleagues from the federal VA Center for Health Equity Research and Promotion found Black and Hispanic Veterans experienced persistent disparities with this type of care, which is VA-funded but delivered outside of the VA health system by non-VA providers.

Approximately 1 in 3 Medicare beneficiaries have an income less than 150% of the federal poverty level (FPL) ($21 870 for an individual in 2023). A complex set of safety-net programs exists to help Medicare beneficiaries with limited means to pay for health care costs, which can be substantial due to premiums, cost sharing, and gaps in Medicare-covered services.
