1 Introduction Medication safety is a global health priority. In 2017, the World Health Organisation (WHO) launched its third global patient safety challenge, aiming to reduce severe, avoidable medication harm by 50% within the next 5 years [1]. A medication-related problem (MRP) is defined as an event or circumstance involving drug therapy that actually or potentially interferes with the patient experiencing an optimum outcome of medical treatment [2].

In 2021, we made a commitment to expand our youth engagement at The Lancet Child & Adolescent Health and launched our inaugural Youth Advisory Panel, with eight youth advisors aged 16–24 years from diverse backgrounds globally. The Youth Advisory Panel sits alongside our International Advisory Board and has been influential in the journal's work in the past 3 years.
