    We thank the author for the letter to the editor based upon our paper “The epidemic of immobility in hospitalized patients: How to get your patient up and moving”. We unreservedly agree that malnutrition is an important issue in hospitalized patients, as well as in many older adults with chronic illness, particularly multimorbidity; contributing to the development of frailty and sarcopenia.     As with many conditions of aging, cardiovascular disease is increasingly common. Ensuring optimal care requires that we adapt care to best support those patients living with frailty. Frailty is a multiply determined state of graded vulnerability to stressors that becomes more prevalent with age. It better predicts risk than does chronological age, and intrinsically affects individuals differently: some chronologically young persons may be frailer than chronologically older ones.
