DiscussionThe main underlying contributing condition among middle-aged individuals reporting breathlessness were respiratory disease (57%) followed by anxiety or depression (51%), obesity (43%) and heart disease or chest pain (35%). Overlap was common with 66% having two or more concurrent contributing conditions. Obesity was associated with reported breathlessness even after adjusting for aerobic fitness level. The present study adds to the previous knowledge in several ways.

Highlights•Liver fat-related genetic variants may or may not raise coronary heart disease risk. •Previous studies have shown different results using different genetic variants. •The effect on apoB-containing lipoproteins may explain previous results. •Here, we were able to directly account for the possible effect on plasma apoB. •We found that the effect on coronary heart disease risk is fully explained by apoB.
