Intercostal artery aneurysms are unusual vascular abnormalities that may lead to life-threatening complications like hemothorax. Hence, it is crucial to emphasize early diagnosis and appropriate management. Furthermore, the challenges of managing such rare cases further increase in limited resource peripheral settings. The current case study highlights the challenges and lessons learned in managing such cases in peripheral settings including the dilemma of ICD insertion.
