Multiple sources briefed on the Defence Flight Safety Bureau's classified interim report have told 60 Minutes that investigators have effectively ruled out mechanical, engine or airframe fault. But analysis of the flight data has uncovered evidence that may suggest sudden and catastrophic pilot disorientation in the moments before the crash, with mounting suspicion that the helmet-mounted navigation visors called TopOwl may have contributed.

And yet David Naggs has found frustratingly few answers as to why his son Alex died, along with three Army aviators, on July 28 last year off Lindeman Island in Queensland. Defence wouldn't even tell David where in the Whitsundays Alex's helicopter had crashed. He'd wanted to at least know the nearest beach so he could visit. Others knew though, such as the first responders to Defence's late night call-out for search and rescue assistance.

The director-general of security used his annual threat assessment on Wednesday to detail how foreign countries are targeting the nation. In an interview with 60 Minutes, The Age and the Sydney Morning Herald, Burgess said there are "dozens" of spies who have been expelled from the country since he took charge. "We're pretty proud of that, and again, we do that quietly with no fanfare, because the key thing is they're put on notice and they're removed and the harm is stopped," Burgess said.
