I was recently asked what I thought were some of the biggest evolving obstacles to physicians and health care in 2024 and beyond. My answer:Health care has become America’s most dangerous profession due to workplace violence, with health care workers now being five times more likely to be assaulted “on the job” than in any other type of workplace in this country. This is one of the most critical obstacles that must be immediately addressed.

It’s capitalism, it’s greed, it’s the hospitals, it’s all those things, but for me, it’s sadness, it’s loss, it’s crying myself to sleep. Lest I sound ungrateful, I am appreciative of the income and career it has afforded me. I am not the victim but also not the perpetrator. In reality, we are all both at one time or another. There are many reasons why doctors, mid-levels, and other health care workers are avoiding rural practice opportunities.

Most of the racially debated issues these days can be summed up by the terms “critical race theory” and “wokeism” – terms that have become the defining issues of our time even though half the people can’t explain them and the other half use them for political gain to dictate how history is taught, stripping it of any mention of slavery, racism, and LGBTQ+ people.

Harry Severance, MD, is a professor of medicine and an advisor on the Healthcare workplace. I recently had a conversation with a self-described early-career, mid-level hospital administrator/manager. This person — who agreed to the interview on the condition of anonymity — reported that in a prior life she was a registered nurse, but moved into Management for a variety of reasons, including less work stress, a better schedule, and a potentially better financial future.

A recent Medpage Today article reviews a research article published in JAMA Internal Medicine. Nidhi Subbaraman, in a topic-related Wall Street Journal article, also reviews this JAMA article and several pilot AI doctor response programs currently undergoing clinical testing. Both of these reviews indicate that AI-created responses to patient-driven online questions posed to doctors are viewed more favorably (by the patients) and rated as having greater empathy than the doctor’s responses.
