This population-based retrospective cohort study targeting all citizens of Japan was conducted using data from the CRoss Icu Searchable Information System (CRISIS), the largest national database of severe COVID-19 patients in Japan (UMIN000041450) [15], combined with data officially published by the Japanese Ministry of Health, Labour and Welfare (MHLW) [16, 17].

In the 2018 Survey of Medical Institutions, the total number of ICU beds per 100,000 population in Japan was 5.4. Of the 339 secondary medical areas in Japan, 199 were excluded because the DPC database covered < 80% of all acute-care beds. The characteristics of the excluded and included patients showed a negligible imbalance in terms of most variables (Supplemental Table 3).

We aimed to evaluate the association between regional critical care capacity and the incidence of IMV for COVID-19 patients in Japan. This population-based cohort study included all Japanese citizens. The data used in this study were obtained from three sources: (i) CRoss Icu Searchable Information System (CRISIS) database; (ii) publicly available databases provided by the Japanese government; and (iii) published data from the Japanese Society of Intensive Care Medicine.
