Dear Editor,Dr Salisbury rightly states that even if the assisted dying bill becomes law, it is still a long time before terminally ill patients potentially have choice over the timing and setting of their death. So, what can we all do now in our clinical practice? Approximately 1% of the population are in the last year of life (accounting for many GP consultations) and almost a third of hospital inpatients are in the last year of life.

What is already known about the topic? •Delirium is highly prevalent and associated with poor outcomes in palliative care. •Evidence-based delirium guidelines are not fully implemented in practice, and their implementation can be challenging. What this paper adds? •Collecting data on delirium outcomes and guideline-adherence from hospice clinical records is feasible. •Demonstrated feasibility of a multi-professional (clinical, domestic and management) implementation strategy.
