This is a secondary analysis of the Comprehensive Post-Acute Stroke Services (COMPASS) study, a cluster-randomized pragmatic trial in North Carolina (NC) examining the effectiveness of a comprehensive transitional care model [9, 10]. The study population was comprised of individuals diagnosed with ischemic stroke, hemorrhagic stroke, or transient ischemic attack (TIA) discharged home from 40 acute care hospitals from 2016–2019.
