We thank Drs Xu and Rong [1] and Drs Leslie and Stranix [2] for their comments regarding our systematic review and meta-analysis exploring the efficacy and safety of intrathecal diamorphine [3]. Drs Xu and Rong correctly highlighted that the figure we initially presented regarding the risk of bias assessment of the included trials was not derived from the Cochrane Collaboration's Risk of Bias tool 2 (RoB2) [1], but rather RoB1.

There is compelling evidence to support the superiority of videolaryngoscopes over direct laryngoscopes for several adult tracheal intubation outcomes [1-3]. However, questions remain regarding this superiority in certain scenarios [4], including whether the results apply to experienced anaesthetists.

Background Nearly half of adult patients undergoing surgery experience moderate or severe postoperative pain. Inadequate pain management hampers postoperative recovery and function and may be associated with adverse outcomes. This multidisciplinary consensus statement provides principles that might aid postoperative recovery, and which should be applied throughout the entire peri-operative pathway by healthcare professionals, institutions and patients.

Nathanson et al. make a case for “career-long mandatory training” for rare but potentially fatal anaesthetic events and that this should be implemented and funded as a matter of urgency [1]. Despite having less than half the reported combined clinical years of anaesthesia experience of the sagacious authors, we echo their sentiment, with a particular focus on mandatory training for airway emergencies.

Introduction Neuraxial administration of hydrophilic opioids, such as morphine, hydromorphone and diamorphine, provides pain relief after surgery [1]. Diamorphine use is limited globally, but evidence suggests it remains widely used in the UK, accounting for nearly 90% of intrathecal opioid use [2]. Intrathecal diamorphine is recommended as standard practice in obstetric settings by the National Institute for Health and Care Excellence in the UK [3].
