Clinical scenario 1A 62-year-old man with a history of rheumatoid arthritis, receiving treatment with methotrexate, leflunomide and plaquenil was referred to the respiratory outpatient clinic for an assessment of a productive cough. He has a past history of pulmonary tuberculosis (TB) and completed directly observed treatment 3 years prior to the referral. He achieved clinical cure; however, he was left with a residual right upper lobe cavity at treatment completion (figure 1a).
