Surveys were completed between March 2022 and July 2023. The international guideline on the definition of PPF [1] was published between distribution of the first and second rounds of the survey. The first survey was sent to 405 physicians in 32 countries and completed by 207, of whom 131 completed the second round and 94 completed the third round. The clinical experience of the respondents is summarised in Table S1 in Additional file 1.

465 subjects met the case criteria for COPD, 213 subjects met the case criteria for IPF, and 348 subjects met the control criteria. Demographic and clinical characteristics of the 1,026 subjects included in our analysis are shown in Table 1. Notably, IPF subjects were predominantly male (70%). The cohort included 90% of self-identified white subjects. COPD subjects were predominantly smokers (95.2% have ever smoked) and IPF and control subjects were 65.3% and 67.8% ever smokers, respectively.

The PF-ILD trial (1199.247, NCT02999178) is a multicentre, prospective, randomised, double-blind, placebo-controlled study to investigate the efficacy and safety of nintedanib in patients with progressive fibrosing lung disease over 52 weeks. The study is conducted at specialised referral centres experienced in ILD management in approximately 15 countries of the Americas, Europe and Asia.
