Key points ○ Cataract surgery is associated with better cognition and lower rates of dementia in United States older adults, though whether the surgery itself improves cognition is unknown. ○ In this cohort of 4384 older cataract surgical patients and 4384 matched controls, cataract surgery was associated with an additional 8 (−10 to 26) days of cognitive aging in the year after, compared with the year before, surgery (p = 0.37).
