What is already known about the topic? •More high quality trials in palliative care are needed to improve the evidence base that underpins clinical practice but trials can struggle to achieve their recruitment targets. •Trial recruitment challenges, including clinician gatekeeping, can be amplified in the palliative care context. •Clinicians have a key role in the recruitment process, but the reasons why they do or do not recruit to palliative care trials is poorly understood.
