MethodsThis research was structured as a retrospective cohort study focusing on diabetic patients residing in City A, a provincial capital city along the eastern coast of China. We designated the years 2014–2017 as the preintervention period, during which we analysed the service utilisation patterns of diabetic patients to establish baseline trends.

Measures of continuity of careIn this study, three indicators of continuity of care were applied, namely, the Continuity of Care Index (COCI), the Usual Provider Continuity Score (UPCS) and the Sequential Continuity of Care Index (SECON), to depict continuity of care from the perspectives of dispersion, density and sequence. It should be noted that, we made some adjustments to the original calculation formulas for these three indicators.
