KAMPALA, UGANDA — The first time CJ was hospitalized for bipolar disorder, she alleges, a staff member who worked in security raped her one evening. She had been admitted at Butabika National Referral Mental Hospital, Uganda’s only national mental hospital, which is about 12 kilometers (7.5 miles) east of Kampala. LUSAKA, ZAMBIA — On a sunny afternoon in Chipungu, a clean-swept hamlet in Rufunsa, a rural district east of Lusaka, three girls who have recently reached puberty sit on the floor of a thatched roof hut in the center of the village. The girls, wearing only their underpants, are seated on a reed mat, their legs stretched out and heads bowed. Around them, women take turns performing sexually suggestive dances, aimed at teaching the teenagers how to engage in sexual acts.

JHAPA, NEPAL — Indira Subedi had always wanted to visit her ancestral home in Nepal. Ten years ago, as she was cutting grass at home in the Indian state of Assam, where she was born and raised, a stray blade pricked her eye. Her vision clouded over; blood clots speckled her eye, making it difficult to open. Someone recommended she seek treatment in Nepal, so she traveled to Birtamod, in the country’s eastern district of Jhapa.

HARARE, ZIMBABWE — Robby, 51, lies on a bed in a public hospital in Harare. A car accident in January resulted in serious leg injuries, and he needed emergency care. At the time of the accident, Robby, who requested to be identified by only his first name for fear of retribution, believed the civil service insurance, which he and his wife had paid close to 20% of their salary into every month since 1997, would cover his bills.
