“The autopsy of the accident site was not done in the way it should have been done. However, the only thing we can say with certainty is that what was officially transported on the trains cannot justify the fireball."

“There was a loss of information necessary to understand the applicants and the factors of the accident,” investigators said.

In addition, HARSIA report establishes that an operational perimeter was not implemented, as well as proper mapping of the accident investigation area.

The investigators said that they took into account all the data, compared the evidence and consider it is "extremely unlikely" that silicone oils caused the fireball.

"There is no indication that the technical equipment involved caused the formation and expansion of the fireball. Simulations and reports indicate the presence of an unknown fuel in the accident," the investigators said.

Regarding the fireball, the report concluded that the number of victims who died from the fire is 5 to 7.

Furthermore, there was no mechanism to monitor the performance of stationmasters, while adding that “the staff performing critical tasks was overworked "beyond human limits", with 2 days off per month.
