A 70-year-old man without previous cardiac history developed sudden onset epigastric pain, nausea, and vomiting while watching television at home. In the emergency department 30 minutes after symptom onset, his blood pressure was 102/75 mmHg, oxygen saturation was normal (98%), and high-sensitivity troponin-I was elevated at 89 859 ng/L (normal <14 ng/L).
