Dear EditorWe should welcome the authors' recognition of the need to treat lived experience input to clinical guidelines with appropriate critical scrutiny - and their proposals for doing so. However, this should not trump other social and economic considerations, expressed through cost-benefit and societal impact analysis. Patient advocacy inevitably stresses the interests of a particular group.
