Dear editorA 45-year-old male presented to an emergency department in the United States (US) 30 min after envenomation by his pet Trimeresurus insularis (). Venom from Trimeresurus insularis causes tissue damage and coagulopathy [Citation1]. No coagulopathy was present on laboratory assessment. Consultation with a poison center and a medical toxicologist was obtained.

Patient with new stroke while on plavix, changed to ticagrelor, believed to be ASCVD, not cardioembolic. While in 90 day DAPT window, now has new DVT (bringing up question as to whether stroke was cardioembolic) Start Apixaban and…. Also, guidance for this anywhere?
