Introduction Patients with type 2 diabetes face heightened risk for the development of comorbid cardiovascular and kidney disease including heart failure (HF).1 The sodium–glucose cotransporter 2 (SGLT2) inhibitor canagliflozin has been shown to reduce the risk of HF hospitalization or cardiovascular death among patients with type 2 diabetes at high cardiovascular and kidney risk across a broad range of patient subgroups in the CANVAS and CREDENCE trials, respectively.2, 3 Loop diuretics are...

Introduction Heart failure (HF) hospitalizations often signal poor prognosis and are associated with significant healthcare expenses.1 Patients with HF with improved ejection fraction (HFimpEF) have a lower risk of HF hospitalizations as compared to patients with HF with reduced ejection fraction (HFrEF),2 but remain at risk for adverse clinical outcomes.3-7 We have previously shown that about 30% of participants in DELIVER (Dapagliflozin Evaluation to Improve the Lives of Patients with...

Introduction Heart failure (HF) and chronic kidney disease (CKD) frequently coexist1 and jointly contribute to increased risk of cardiovascular (CV) events, progression of kidney disease, and mortality.2 The recently released 2024 Kidney Disease Improving Global Outcomes (KDIGO) clinical practice guidelines strongly underscore the importance of robust estimation of kidney risk.3 Both lower estimated glomerular filtration rate (eGFR) values and higher levels of albuminuria predict adverse...

Background: Hospitalization is recognized as a sentinel event in the disease trajectory of patients with heart failure (HF), but not all patients experiencing clinical decompensation are ultimately hospitalized. Outpatient intensification of diuretics is common in response to symptoms of worsening HF, yet its prognostic and clinical relevance, specifically in patient with HF with mildly reduced or preserved ejection fraction is uncertain.
