The 2023 report of the Lancet Countdown on health and climate change confirms climate change as the biggest global health threat of the 21st century.Although the findings of the 2023 report are not surprising, they are incredibly worrying for the survival of humanity. We are currently experiencing a mean heating of 1·14°C above pre-industrial levels, and policies to date put the world on track to almost 3°C of heating.

This paper explored the institutional conditions that shape health sector actors’ ability to protect public health interests in tobacco governance in Fiji and Vanuatu. Our study contributes to the commercial determinants of health, health governance and development literature by shedding light on the connection between SIDS vulnerabilities, political context, and government structures, rules, and accountability in tobacco governance in Fiji and Vanuatu.
