Key points ○ In this longitudinal analysis of 94 million Medicare discharges, between 2010 and 2022, primary care follow-up after return to home increased from 51.5% to 57.5% for patients discharged directly home and from 24.3% to 28.4% for patients discharged to skilled nursing facilities (SNF) then home.
