Scottsdale, AZ-based outpatient medical imaging provider SimonMed Imaging has confirmed that it experienced a data breach in January and will issue notices about the event to affected individuals. The company was alerted by one of its vendors of a "security incident" on January 27. It reviewed its systems, discovered suspicious activity, and began an investigation. The investigation revealed that there indeed was "unauthorized access to [the company's] systems between January 21 and February 5.
