Dear Editor,We read with interest the article regarding human factors and team working which brought up a number of interesting points. It alluded to the concepts of identifying where intensely focusing on certain areas is useful (the well known ‘safety I’ perspective- ensuring as few things as possible go wrong); and how human performance practically always goes right in spite of no one being perfect (the newer ‘safety II’ approach- enabling as many things as possible to go right)[1].
