    IntroductionWhen I (J.R.H.) started my residency at Comer Children's Hospital in 1977, my colleague, Dr. Joel Schwab, a general pediatrician at the University of Chicago, was treating a 3-year-old patient who presented with a presumptive diagnosis of acute sinusitis. She had a fever, headache, and a purulent nasal discharge for 1 week. She did not have any clear sinus tenderness at the time of examination, which led to a discussion about an acute bacterial sinusitis (ABS) diagnosis.
