A 52-year-old man was referred to our team for assessment of a mass in the right atrium. The lesion, which initially appeared to be an extending thrombus in the inferior vena cava, had been seen on CT scan 6 months earlier as part of systematic surveillance for carcinoma of the colon (figure; appendix). He had been started on low molecular-weight heparin and re-commenced on both nivolumab and ipilumumab. This article is available free of charge.

A 45-year-old woman presented with diplopia and vertigo. Brain MRI showed cerebellar bilateral ischemic lesions. Transoesophageal echocardiography showed thickening of the leaflets of the aortic valve and mild aortic regurgitation. CT scan showed a left inferior lung mass and left supraclavicular lymphadenopathy. After lymphadenopathy puncture, the patient was diagnosed with an ALK fusion driven lung adenocarcinoma.
