People who love the long-term care profession may feel like suckers sometimes. How does America’s centrally planned, heavily regulated, top-down LTC system treat us like chumps? It would be hard to have any kind of LTC business without accepting Medicaid. But Medicaid pays less than the cost of providing care, on average. You can’t make up for losses like that by serving more people on public assistance.

Before Medicaid came on the scene in 1965, everyone paid out of pocket for long-term care (LTC). Operators received a market rate for the services they provided. There was no special rate below the cost of care for public welfare recipients. Nor was there a special, higher private-pay rate to shift uncompensated costs to private payers. Those were the days!Then came Medicaid.

Most of long-term care’s problems — including impaired access and quality, institutional bias, interminable home-care waits, paid caregiver shortages, and excessive dependence on unpaid family and friends — are the direct result of too much reliance (61% of $400 billion in 2022 LTC spending) on Medicaid’s parsimonious reimbursements (70% of private-pay rates) and the Centers for Medicare & Medicaid Services’ overbearing regulation (such as uncompensated staffing mandates).
