This commentary uses the terms “queer” and “Hispanic/Latino.” The terms are chosen to encompass a spectrum of non-heterosexual orientations, providing broader perspective on the issues discussed, and to resonate with a wide demographic while acknowledging the diversity within these communities. Additionally, “MSM” (men who have sex with men) is utilized in line with health care research to address specific health dynamics without assuming sexual orientation.

Health care costs in this country continue to rise without a commensurate increase in quality. These increased costs squeeze public budgets and individual and family pocketbooks, including through higher health insurance premiums. Many states are interested in addressing these rising costs and have multiple tools available to do so.

Burnout and stress among nurses is an acute issue. Almost one-third of registered nurses and two-thirds of critical care nurses say that they are planning to leave their direct patient care position in the next year with burnout being a major contributing factor.

Health care in this country is unaffordable, even for people who have insurance coverage. About half of insured adults in the United States have difficulty affording the health care they need. A 2022 poll by the Kaiser Family Foundation found that 47 percent of insured, non-elderly adults find it somewhat or very difficult to afford health care costs.

One of the Affordable Care Act (ACA)’s major provisions for expanding access to affordable health coverage was the creation of health insurance marketplaces, along with subsidies to promote affordable coverage. Open enrollment for marketplace coverage for 2024 begins on November 1, 2023 in most of the country.
