    Preventable or unpreventable? Any time there is an investigation into a health-care staff member’s injury or illness, that’s how the inquiry begins. As a national representative with Canada’s largest union for 20 years, my assignments included health and safety investigations at hospitals, long-term-care residences and paramedic work sites. All of the reviews would conclude these tragedies are preventable.
