This scientific commentary refers to ‘Late-onset temporal lobe epilepsy: insights from brain atrophy and Alzheimer’s disease biomarkers’ by Ballerini et al. (https://doi.org/10.1093/brain/awae207). The bidirectional relationship between late-onset epilepsy and dementia has recently been the focus of intense research.1 Older individuals with epilepsy have an increased risk of developing dementia, which raises significant concerns given the ageing global population.
