The last three decades have seen a sharp increase in the number of cirrhosis deaths in the United States, with figures reported by The Lancet Public Health in August 2024 showing an uptick of 87% between 1990 and 2019.

    A 54-year-old woman is evaluated in the emergency department 3 hours after a 30-minute episode of chest discomfort. She is currently pain free. She reports a 2-day history of recurrent episodes of chest “fullness” while at rest. Episodes are 7/10 in severity and last 5 to 45 minutes without radiation or other associated symptoms. She also reports discomfort after meals. She has not exerted herself because of concerns of precipitating chest discomfort.

    Thank you for the excellent article on falls (“Age and the Rising Risk of Falling,” January 2025). However, I have a concern—perhaps it's a soapbox I've been on for a while. The article includes at least four references to the time limitations of primary care visits.
