BackgroundSelf-reported health (SRH) is a widely used health indicator in surveys and questionnaires. Questions such as ‘How is your health in general?’ followed by response options of ‘Very good’, ‘Good’, ‘Fair’, ‘Bad’, or ‘Very bad’ are included in many questionnaires. The SRH measure gained attention when research identified its association with mortality in the 1970s and 1980s [1].

IntroductionWe need hardly mention that the notion of a connection in its various forms is of vital importance in differential geometry and geometric approaches to physics (see, for example, [1]). As an important example of the role of connections in modern mathematics, we point to the construction of characteristic classes of principal bundles via Chern–Weil theory (for an introduction, see [2], Chapter 11).
